What is a Schedule of Benefits (SOB)?

This document provides essential information about a health insurance plan, including costs, benefits, and exclusions. Also referred to as a Summary of Benefits and Coverage (SBC), it outlines:

  • What a health plan covers and doesn't cover
  • Deductibles, out-of-pocket maximums, copays, coinsurance
  • The costs associated with each type of service
